-HPI 48 yo f c/o chest pain. -onset a few hours ago, nonprogressive, no prior hx - 16h airplane flight -sharp chest pain, 6/10, nonradiating, worse with breathing -denied fever, chills, cough, n/v, abdominal pain -alleviates with acetaminophen -aggravates with breathing and moving ROS: neg except as above PMH: none / migraines for 2 yrs / asthma good control -appendectomy/cholecystectomy/C-section at 16 yo -allergies: NKDA / PCN – rash, itching of the skin -meds: multivitamins / OCP / no Rx or OTC -FH: noncontributory SxH: sexually active with multipleF partners / uses condoms all the time / OCP for contraception / inconsistent use of condoms / no h/o of STDs -LMP 2 wks ago, 6/30, requiring 6 pads per day, NL Pap last yr -uncomplicated NSVD (Normal Spontaneous Vaginal Delivery) at full term 5 yrs ago -SH: 1 PPD x25 yrs, no/occasional smoking /ETOH /rec drugs -works as engineer / retired -single/married for 5 yrs |
